Care Coordinator


We are looking for a Care Coordinator to join our busy team in Trafford. 

The successful candidate must hold a full UK driving licence and have a good geographical knowledge of Trafford and the surrounding areas.Experience of working in a busy office environment is a must, as is having experience in the health and social care sector. The ideal candidate must hold an NVQ2 or higher in health and social care.

Computer literate, you will be able to work effectively as part of a team. 

Excellent customer service skills are essential, as there is a requirement to liaise with a variety of health care professionals. 

Self-motivation and the ability to effectively manage and prioritise workloads are necessary. 

Responsibility for weekly rotas, along with visiting clients in their own homes to complete care plans and risk assessments, are integral to the Care Coordinator role.

Hours are 9am to 4pm, Monday to Friday. 

The successful candidate must be willing to share the on call, on a rolling rota. 

20 days paid holiday, plus bank holidays are associated with this role.

There will be occasions when the Care Coordinator will be expected to carry out hands on care work in the community. 

The successful candidate will be required to complete an enhanced DBS check, the expense to be met by the employer.

Salary for this position is negotiable depending upon experience.
